Collect 100 hand slot appearances.

Collect 100 waist slot appearances.

Collect 100 head slot appearances.

Collect 100 shoulder slot appearances.

Equip an epic item in every slot with a minimum item level of 840.

Collect 100 feet slot appearances.

Defeat the Shade of Xavius in Darkheart Thicket on Mythic difficulty.
